Job description

Employment Type: Full time Shift: Day Shift Description: Hospice Admissions & Intake RN Manager Community Hospice | Albany, NY Area Community Hospice has served the Capital Region for decades, and we're looking for an RN leader ready to make an immediate impact on patient care in Albany and the surrounding communities. This is your chance to lead a high-performing Hospice Intake and Admissions team at an organization that's investing in growth, innovation, and the people who make hospice care possible. The Hospice Admissions & Intake RN Manager oversees hospice admissions, referral intake, eligibility review, intake coordination, and admission operations across the service area. We're seeking a high-energy, fast-paced leader who thrives in a dynamic, growth-oriented environment and can drive both operational and cultural transformation for our Hospice Intake and Admissions team. What You Will Do • Provide administrative direction, clinical oversight, and daily operational leadership for Intake and Admissions across the Hospice service area. • Build and reinforce a culture where speed and responsiveness to referrals is critical, providing support and assistance to ensure an easy, fast intake process. • Ensure rapid turnaround for admissions, recognizing that patients are often referred late and time is a key factor in care outcomes. • Arrange timely admissions, ensuring accessibility 365 days a year. • Partner collaboratively with Provider Relations on the education of healthcare professionals, referral sources, and the general public regarding hospice care and eligibility. • Ensure compliance with regulations, accreditation standards, and agency policies and processes. • Serve as a member of the Community Hospice senior leadership team, helping shape strategy for the organization's future. What You Will Need • A current license to practice as a Registered Nurse in the State of New York. • BSN preferred but not required. • Prior supervisory experience strongly preferred. • Minimum of three (3) years hospice, acute care, or homecare RN experience required. • Comfort providing direct oversight of the intake department and stepping in to complete a hospice admission when needed. • Prior hospice experience preferred. Why Join Community Hospice Albany and the greater Capital Region offer the best of both worlds: an affordable cost of living, easy commutes, excellent schools, and quick access to the Adirondacks, the Hudson Valley, and the Berkshires for weekend getaways. It's a place to build a career and a life. Community Hospice backs that up with real support for our team: • A thorough orientation, dedicated preceptors, and a genuinely supportive work environment from day one. • Hands-on training in end-of-life care, so you're set up to lead with confidence. • A senior leadership seat where your voice shapes how we serve patients and families across the region. • Competitive pay: $48.00–$63.38/hour, based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) are paid within the base salary equivalent of the stated hourly rates. Pay range may vary within the stated range based on location.
