Rosemount is a great opportunity for families seeking a balance between small-town charm and growth opportunity .        Practice Details:   • 11 provider primary care practice which includes Family Medicine, Internal Medicine, Pediatrics, and Nurse Practitioners and Physician Assistants    • Specialists in Behavioral Health Consultant, Certified Diabetes Educator, anticoagulation, medication therapy management, and an onsite pharmacy.     • Patient encounters average 18-20   patients per day.   • Virtual care offered via video, telephone, and through EMR.    • 34 patient contact hours per week for full time - flexible schedule.    • Call Frequency: 1:6 low intensity - phone consultation only with first tier nurse triage   • Employer-paid Nabla AI clinical documentation support to reduce administrative burden and streamline charting.   • As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.   • Fully integrated Electronic Medical Record (EPIC).     M Health Fairview Clinic – Rosemount is located  25 minutes from both Minneapolis and Saint Paul.
